How to Prepare a Clear HVAC Service Request

A complete service request helps the business understand what equipment is involved and what kind of visit you are asking about. These notes are useful for heating, cooling, plumbing, and commercial refrigeration requests throughout Central Maine.

Identify the equipment and the work you need

Name the system type and, if available, its make and model. Say whether the request is for installation planning, a current repair, or routine maintenance; one word such as “heating” may refer to different equipment and work.

  • Equipment type and model, if known
  • Installation, repair, or maintenance
  • Town and building type
  • Any important access restrictions

Describe the symptom as it happens

Say when the problem began, how often it happens, what a control displays, and which rooms, fixtures, or equipment zones are affected. For commercial refrigeration, include an observed temperature and any steps taken to protect stored product under your own safety procedures.

Share safe, relevant photos and context

A clear photo of a model label, control display, or visible equipment area may help explain the request. Avoid opening covers or photographing people, account details, or unrelated private areas. The MD HVAC & Plumbing contact form can attach up to three equipment photos to the email sent to the business.

Confirm the reply path and scheduling expectations

Provide either a phone number or an email address for the business to reply. A web form does not reserve a time or provide emergency dispatch; scheduling and scope are confirmed separately. For an unsafe condition, contact the appropriate emergency service instead of waiting for an online reply.
